Position Objection:
The Health Outreach Program Manager will be responsible for maintaining relationships with ES clients, and for the success of the Health Outreach Health Worker Program. Β The Program Manager will oversee the day-to-day operations of a staff of Community Health Workers (CHWs) and doulas assigned to the client project; collaborates with stakeholders via direct communication, reviews online surveys or comments, secondary escalations complaints and opportunities on behalf of the client; Facilitates between client, scope of service performance and service delivery.Β
Position Responsibilities:
- Manages day-to-day operations of Health Outreach Community Health Worker Program.Β
- The Program Manager maintains accountability for the achievement of program(s) goals and staff compliance with organization, client goals and deadlines (e.g., certifications, trainings, standards, and benchmarks)
- Develops and leverages community partner relationships within and across constituencies, identifying opportunities to secure community partner action plans for expansion of association programs and services
- Hires, and trains new staff and provides ongoing training and refreshers to team as needed
- Supervises staff according to company policies, monitors direct staff and their hours ensuring effective staffing
- Establishes and manages performance expectations, mitigates team conflict and communication issues
- Operates as a part of the team by providing program coverage as needed
- Demonstrates effective management as evidenced by timely and thorough follow-through on assignments
- Participates in the development, implementation and evaluation of goals, objectives, and policies for the outreach staff
- The Program Manager assists in the development and administration of the assigned program budget, monitors and approves expenditures and recommends adjustments as necessary
- Conducts research into programs designed to address the target population and community outreach programs
- Develops and conducts analysis of data for preparing reports to better support implementation
- Conducts ride-a-long assessments with remote staff while utilizing various job coaching techniques
- Other duties as assigned
Minimum requirements:
**Maternal Child Health Experience Required. Clinical Experience Required**
- Bachelor's degree in social work, Health Administration or a closely related field is highly preferredΒ
- Two β four years of experience in marketing, community outreach and/or human services planning and development. Or Four (4) years of related experience; or any combination of education and experience, which would provide an equivalent backgroundΒ
- Two years supervisory experience preferred
- Working knowledge of community-based populations, medical conditions and/or disabilities
- Experience working with maternal and child populations; preferred clinical experience with populations
- Medical Management BackgroundΒ
- Medicaid Managed Care Experience
- Quality/HEDIS Experience preferred
- Case Management Experience preferred
- Ability to travel and conduct territory visits, as needed
- Ability to train staff with strong presentation skills
- Strong Analytical Skills to include performance reports and scorecards
- Experience managing remote teams
- Must have the ability to obtain a valid Indiana Driverβs License and have access to reliable transportation
- Must be willing to develop relationships within local communities and have strong presentation, interpersonal and communication skills
- Must have good organizational skills and be able to work independently and effectivelyΒ
- Ability to handle protected health information and personal identifiable information in accordance with HIPAA requirements. Β

Physical Demands/Work Environment:
The position can range from sedentary with long period of sitting and working at a computer or driving various distances. Β Manual dexterity is required to type and operate office equipment. Β When attending events, you may be required to stand or walk for a long period of time. Β Ability to lift to 25 pounds.
The worker must have close visual acuity to perform an activity such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading. Must be licensed to operate a motor vehicle which requires visual acuity. Β
The position requires the individuals to obtain and exchange information and assist the staff with resolving simple to complex issues. Β The work requires frequent communication, requires the ability to express self and receive information. Β Must also have dedicated workspace to protect member information
